Understanding Menopause and Current Guidelines

According to recommendations from the North American Menopause Society (NAMS), the American College of Obstetricians and Gynecologists (ACOG), and the Endocrine Society, menopause care should be individualized based on symptoms, medical history, risk factors, and personal health goals.

Menopause is typically diagnosed after 12 consecutive months without a menstrual period in women who are not using hormonal contraception. The transition leading up to menopause, known as perimenopause, may involve fluctuating hormone levels and symptoms that can significantly affect daily life.

Why Menopause Management Matters

Declining estrogen levels during menopause can contribute to important health changes, including:

  • Cardiovascular health: Estrogen decline is associated with changes in cholesterol levels, blood vessels, and cardiovascular risk.
  • Bone health: Reduced estrogen increases bone loss and the risk of osteoporosis and fractures.

Hormone Replacement Therapy (HRT) for Menopause Symptoms

Hormone replacement therapy (HRT), also called menopausal hormone therapy (MHT), remains the most effective treatment for moderate to severe menopause symptoms, including:

  • Hot flashes and night sweats (vasomotor symptoms)
  • Sleep disruption related to menopause symptoms
  • Menopause-related vaginal and urinary symptoms

Current guidelines from NAMS and ACOG support the use of HRT for appropriately selected women, particularly those who are younger than age 60 or within 10 years of menopause onset, when the benefits generally outweigh the risks.

Treatment decisions should be individualized and consider age and time since menopause, personal and family medical history, risk of cardiovascular disease, blood clots, breast cancer, and osteoporosis, and type, dose, and route of hormone therapy.

For women with a uterus, estrogen therapy is typically combined with a progestogen to reduce the risk of endometrial cancer. Local vaginal estrogen therapy may be considered for persistent genitourinary symptoms with minimal systemic absorption.

Healthy Lifestyle Is the Foundation of Menopause Care

Regardless of whether hormone therapy is used, lifestyle choices play a major role in maintaining health during and after menopause:

  • Regular exercise including resistance training and weight-bearing activity to support bone and cardiovascular health
  • Heart-healthy nutrition emphasizing vegetables, fruits, lean proteins, whole grains, and adequate calcium and vitamin D
  • Maintaining a healthy weight
  • Avoiding tobacco and limiting alcohol
  • Regular screening including blood pressure, cholesterol, diabetes screening, bone density testing when appropriate, and age-appropriate cancer screenings
